TRANSITION SERVICES DIAL PROJECT

Transition Services Dial Project is a revolutionary project partly funded by the California State Department of Rehabilitation to create a bridge for people with disabilities from hospital, skilled nursing facility, or institutional facility to home.

Craig, 51, paralyzed from the neck down, found himself at *Rancho Los Amigos. For the next year and a half, he underwent surgeries, each followed by months of rehab therapy. His paralysis was now from the hips down and he knew he could live on his own again. Talking to people at Rancho, he heard of the Transition Services Project at WCIL and contacted us.

Collaborating with the WCIL Housing Specialist and Benefits Specialist, transition staff was able to arrange and find a low-rent apartment using Craig's new Supplemental Security Income (SSI). Next they helped him find a new physician nearby his apartment, and arrange for an occupational therapist to come visit and perform a home safety and modification assessment. Now, all is well. Craig has been successfully living in his own apartment for several months, been evaluated by the Department of Rehabilitation for employment training and is now focusing on his final goal of full-time employment as a computer technician.

*Rancho Los Amigos and WCIL staff have a direct referral arrangement which can, when chosen by the consumer, make housing arrangements instead of/or prior to entering a skilled nursing facility.
